Surprisingly, before they take their last breath, they normally get the strength of about 10,000 gorillas to still be able to take a last journey to the travel destination of choice for terminally ill patients which also goes by the name of THE BEACH.
There, they are shown being as pale as ghosts but with a weak smile on their faces as they are piggybacked by the man they love while watching the sunset, which coincidentally also seems to be the universal code for them to take their final breath while serving as a reminder for the male lead that succeedingly, he has to throw himself under a fast moving vehicle a few days after.
